Why Payer AI Needs Receipts

From Denial Rates to Evidence Packs

For years, prior authorization and payer decision-making have lived in a “gray zone.”

Everyone knew the friction was real. Providers felt it. Patients experienced it. Payers managed it. Vendors promised to “optimize” it. But for too long, the core of the conversation stayed hidden behind fragmented workflows, call centers, black-box rules, and internal dashboards.

That era is ending.

As of today, March 31, 2026, the landscape has fundamentally shifted. CMS now requires impacted payers to publicly report prior authorization metrics, approval rates, denial rates, approvals after appeal, and turnaround times. With tighter decision timelines (72 hours for urgent, 7 days for standard), prior authorization is no longer just an internal utilization management process. It is a visible, measurable signal of operational quality and institutional trust.

And right on cue, the next question has arrived:

If AI is increasingly involved in payer decisions, where are the receipts?

The End of the Black Box

This isn’t a theoretical debate anymore. It is legal, operational, and reputational.

Take, for example, the Electronic Frontier Foundation’s recent FOIA lawsuit against CMS regarding the WISeR program. The concerns, accuracy, bias, and a lack of public transparency highlight a growing consensus: in healthcare, automation without explainability is becoming unacceptable.

Whether you are an AI optimist or a skeptic, the mandate is the same. The future of payer AI will not be defined by who has the biggest model or the flashiest demo.

It will be defined by who can prove the decision.

In healthcare payments, a prediction is not enough. A confidence score is not enough. If a claim is denied, downcoded, or routed for intervention, the payer must be able to show why. Not in abstract language or probabilistic guesses, but by pointing to the specific policy clause, the contract logic, the clinical fact pattern, and the reasoning chain that led to the outcome.

The Problem with “Consumer-Grade” AI in Healthcare

For too long, healthcare automation has borrowed its mental model from consumer AI: predict the next thing, rank the next action, optimize the funnel.

But claims adjudication and medical necessity are not ad-targeting problems. They are governed decision systems. They require determinism, lineage, and auditability.

Healthcare isn’t asking, “Can your model be clever?” Healthcare is asking, “Can your system stand up in an audit, a provider dispute, a regulator review, and a courtroom?”

Those are two very different requirements.

Why Neuro-Symbolic is the Solution

Earlier this month, the DOJ announced that Aetna agreed to pay $117.7 million to resolve allegations tied to inaccurate Medicare Advantage diagnosis submissions. Simultaneously, KFF analysis shows significant disruption in the MA market, particularly in rural areas.

These stories point to one truth: payer economics are under pressure, and administrative decisions now carry much higher stakes. In this environment, “faster AI” isn’t a winning strategy. “Provable AI” is.

The Rise of the “Evidence Pack”

The next generation of payer platforms must do more than detect anomalies. They need to produce what I call an Evidence Pack: a glass-box artifact that shows:

  • The source clause.
  • The extracted clinical facts.
  • The applicable reimbursement logic.
  • The exact rationale for the final action.

This is how we move from post-pay recovery to pre-pay confidence. It allows SIU teams, claims teams, and medical policy directors to work from a single, unassailable source of truth.

Scaling Trust

We are moving from a world where payers could simply make decisions to a world where they must explain them.

Public denial-rate reporting is one signal. AI transparency lawsuits are another. Risk-adjustment enforcement is a third. Put them together, and the message is clear: every important payer decision must be measurable, reviewable, and defensible.

The winners in this space won’t be the ones with the best black box. They will be the ones with the best receipts.
